of Student Intern form. Use the correct from for the student’s
major.
In completing the form be sure to include
the student’s name, major, and internship cycle. We
use these ratings and comments to determine the employer portion
of the internship grade.
Fax the completed form to the attention of your LaGuardia
internship faculty contact.
